본 연구는 우리나라 노인 인구의 신체활동 참여와 건강검진 이용이 건강관 련 삶의 질에 미치는 영향을 분석하고자 수행되었다. 연구에는 국민건강영양조 사 제9기 2차년도(2023) 자료를 활용하였으며, 만 65세 이상 노인 1,728명을 최 종 분석 대상으로 선정하였다. 종속변수는 한국형 건강관련 삶의 질 측정 도구 인 HINT-8 지수였으며, 주요 독립변수로는 일반적 특성, 신체활동(고강도, 중강 도, 걷기), 건강검진(일반검진, 암검진)을 포함하였다. 분석 방법으로는 2단계 층화집락표본설계를 반영한 복합표본분석을 적용하 였고, 차이분석(t-검정 및 ANOVA)과 복합표본 다중회귀분석을 병행하였다. 분석 결과, 성별, 연령, 결혼, 교육, 소득, 경제활동, 주관적 건강인지, 활동제한, 음주, 근력운동, 유산소운동 등 일반적 특성에 따라 HINT-8 지수에 유의한 차이가 있 는 것으로 나타났다. 남성, 연령이 낮은 집단(65~69세), 유배우자, 고학력자, 경제 활동 참여자, 활동제한이 없는 자, 비음주자, 근력운동 및 유산소운동 참여자, 주관적 건강인지가 긍정적인 자 등에서 삶의 질이 높았다. 신체활동 측면에서는 걷기, 중강도 신체활동 참여자들이 비참여자 보다 유의하게 높은 삶의 질을 보 였으나, 건강검진 여부에서는 삶의 질 차이가 나타나지 않았다. 다중회귀분석 결과, 고강도 신체활동 및 걷기 참여가 삶의 질에 긍정적인 영 향을 미쳤으나 중강도 신체활동과 건강검진은 회귀모형에서 통계적으로 유의한 변수로 나타나지 않았다. 본 연구 결과, 신체활동 중 고강도 신체활동과 걷기는 노인의 건강관련 삶의 질에 통계적으로 유의한 긍정적인 영향을 미치는 반면, 건강검진은 유의한 영향 요인으로 나타나지 않았다. 이는 건강검진 자체보다는 개인의 주관적 건강인지 와 기능적 상태가 삶의 질에 더 밀접한 영향을 미칠 수 있음을 시사한다. 이러 한 결과를 바탕으로 해당 요인을 반영한 맞춤형 건강증진 프로그램 개발이 요구된다.

This study aimed to analyze the effects of participation in physical activity and utilization of health examinations on the health-related quality of life (HRQoL) among the elderly population in South Korea. The data were derived from the 2nd year of the 9th Korea National Health and Nutrition Examination Survey (KNHANES, 2023), with a final sample of 1,728 individuals aged 65 and older. The dependent variable was the HINT-8 index, a Korean-specific measure of HRQoL, while the main independent variables included general characteristics, physical activity (vigorous, moderate, and walking), and health examination (general and cancer screenings).
A complex sample analysis reflecting the two-stage stratified cluster sampling design was employed. Differences were examined using t-tests and ANOVA, and multiple regression analysis was conducted. The analysis revealed that HRQoL significantly differed according to various general characteristics, including sex, age, marital status, education level, income level, economic activity, perceived health status, activity limitation, alcohol consumption, muscle-strengthening activity, and aerobic activity. Higher HRQoL scores were observed among males, younger elderly (65–69), those with spouses, higher education levels, economically active individuals, those without activity limitations, non-drinkers, participants in muscle-strengthening and aerobic exercises, and those with positive self-perceived health. Regarding physical activity, those who engaged in walking and moderate activity showed significantly higher HRQoL compared to non-participants, while no significant differences were found in HRQoL based on health examination participation.
Multiple regression analysis indicated that participation in vigorous physical activity and walking had a statistically significant positive impact on HRQoL, whereas moderate physical activity and health examination participation did not show statistically significant effects.
In conclusion, vigorous physical activity and walking were found to positively influence HRQoL in older adults, whereas participation in health examinations was not a significant predictor. These findings suggest that subjective health perception and functional status may have a more direct impact on HRQoL than health examinations per se, indicating the need for tailored health promotion programs that incorporate these factors.
