본 연구는 임플란트 식립과 동반된 골유도재생술에서 돼지 유래 이종골 이식재의 유효성 평가를 목적으로 소 유래 이종골 이식재와 비교하여 시행된 전향적, 단일 맹검, 무작위 배정 대조 임상 연구이다.
모든 재료와 연구 방법은 부산대학교 치과병원의 임상시험 심사위원회 (IRB 승인 번호: PNUDH-2019-007-MD)의 승인 하에 진행되었다. 부산대학교 치과병원에 내원한 환자들을 대상으로 시행된 적합성 평가 후, 하나 이상의 치아를 상실한지 6주 이상 경과되고, 협측에 2-3 mm의 치조골 결손부를 지닌 총 38명의 피험자들이 선택되었다. 이들은 적용될 골이식재의 종류에 따라 각각 19명씩 대조군 (소 유래 이종골 이식재, Bio-Oss군)과 실험군 (돼지 유래 이종골 이식재, Bone-XP군)으로 무작위 배정되었다. 각 군에서는1차 수술 시에 임플란트 식립과 함께 골 결손부에 서로 다른 종류의 이종골 이식재가 적용된 골유도재생술이 시행되었고, 술 후 6개월의 2차 수술 시까지의 추적관찰이 진행되었다. 이번 임상 연구에서 돼지 유래 이종골 이식재의 유효성 평가는 골결손부 크기 변화에 따른 골재생능 (bone regeneration)과 이식재의 체적안정성 (volumetric stability), 임플란트 고정체의 안정성 (implant stability), 그리고 임상적 안전성 (clinical safety) 측면에서 소 유래 이종골 이식재와 비교되어 실시되었다. 이를 위해 임플란트 1차 수술 시기와 2차 수술 시기에 다음의 항목들이 각각 측정되었다. 골재생능 평가를 위해 치조골 협측 결손부의 수직 길이와 수평 길이 및 근원심측 결손부의 수평 길이가 측정되었다. 체적안정성의 평가를 위해 협측 결손부에 위치된 골이식재의 협측 수평 두께가 측정되었으며, 추가적인3차원적인 평가를 위해 1차 수술 전, 임플란트 식립과 동반된 골유도재생술 직후, 그리고 2차 수술 시기에 cone-beam computed tomography (CBCT)를 이용한 방사선 사진을 촬영하였다. 방사선 사진의 digital imaging and communications in medicine (DICOM) 파일을 소프트웨어 상에서 stereolithography (STL) 파일로 전환하여 관심영역 (region of interest; ROI)에서의 골부피를 측정하였다. 임플란트 고정체의 안정성 평가를 위해 임플란트 안정성 지수 (implant stability quotient; ISQ)가 측정되었다. 모든 평가 항목들은 시기에 따른 변화량으로 정량화되고 대조군과 실험군 간의 유의미한 차이에 관하여 분석되었다. 임상적 안정성은 환자들의 술 후 1개월, 3개월 그리고 6개월 내원 시기에 임상검사 및 이상반응 발생 여부를 확인하여 평가되었다.
본 임상 연구의 결과 협측 결손부 수직 길이의 변화량은 대조군과 실험군에서 각각 2.09 ± 0.36 mm, 실험군에서 2.06 ± 0.54 mm로 측정되었으며, 두 그룹간 유의한 차이는 나타나지 않았다 (p > 0.05). 협측 결손부 수평 길이의 변화량에서도 각각 1.71 ± 1.16 mm, 1.74 ± 1.23 mm로 유의한 차이는 나타나지 않았다 (p > 0.05). 근원심측 골결손부 수평 길이의 변화량은 대조군과 실험군 각각 1.26 ± 0.73 mm, 1.18 ± 0.97 mm로 측정되었으며, 두 그룹간 유의성 있는 차이는 보이지 않았다 (p > 0.05). 골 결손부에 적용된 골이식재의 협측 수평 두께 변화량 또한 각각 0.97 ± 0.57 mm, 0.94 ± 0.73 mm로 유의한 차이는 관찰되지 않았다 (p > 0.05). 임플란트 안정성 수치의 변화량 차이에 있어서도 유의한 차이를 보이지 않았으며 (p > 0.05), CBCT영상 분석을 통한 3차원적 평가에서도 두 그룹이 유사한 골이식재 유지율을 보였다 (p > 0.05). 추적관찰 기간 동안의 임상적 이상 반응 평가에서는 대조군에서 심근경색증으로 인한 1명의 탈락자가 발생하였으나 그 외에 환자들에게서 이상 반응은 관찰되지 않았다.
이와 같이 모든 측정치에 있어 두 군 간에 통계학적으로 유의미한 차이는 없었으며, 실험군이 대조군과 유사한 임상적 효과를 나타냄을 확인하였다. 따라서 본 연구 결과에 한하여 돼지 유래 이종골 이식재는 소 유래 이종골 이식재의 적합한 대체재로서 임상에 유용하게 적용될 수 있다고 볼 수 있을 것이다.
