    배경 및 연구목적
    진주종은 완전한 절제를 포함한 수술적인 치료에도 이비인후과 영역에서 정복하지 못한 질환이다. 진주종의 병태생리에 대한 정확한 이해가 아직 부족하며, 관련된 마이크로 RNA에 대한 연구 또한 부족한 실정이다. 본 연구는 차세대 염기서열 분석 (NGS)을 통하여 정상 피부조직과 중이 진주종의 마이크로 RNA 발현을 비교 분석한 연구이다. 진주종의 마이크로 RNA 프로파일과 이와 연관된 분자생물학적 경로에 대해 조사하였다.

    연구대상 및 방법
    중이 진주종 및 후이개 피부조직 검체를 중이 수술 중인 성인 환자 13명으로부터 채취하였다. 총 RNA를 추출하고 NGS를 이용하여 마이크로 RNA 발현 프로파일을 분석하였다. 대상 유전자를 예측하고 관련 생물학적 경로를 예측하기 위해 DIANA-microT-CDS와 the Kyoto Encyclopedia Gene and Genome database (KEGG) 경로를 사용한 기능적 유전자 분류를 시행하였다.

    결과
    중이 진주종 및 피부 조직 검체로부터 총 2,588 개의 miRNA 발현을 확인했다. 중이염 검체에서 정상 피부 검체와 비교하여 76개의 상향조절 마이크로 RNA와 128개의 하향조절 마이크로 RNA의 발현을 확인했다 (FC≥2 및 p <0.05). 진주종의 병태생리와 관련된 생물학적 경로를 확인하기 위해 99개의 마이크로 RNA (FC≥4 및 p <0.05)를 사용했다. 상향조절 마이크로 RNA 그룹에서는 ErbB 신호 경로가, 하향조절 그룹에서는 ECM 수용체 상호작용이 가장 연관성 높은 경로임을 확인했다.

    결론
    본 연구는 NGS 기술을 활용하여 후천성 진주종의 마이크로 RNA를 분석한 첫 번째 연구이다. 우리는 진주종과 연관된 새로운 miRNA와 생물학적 경로에 대해 확인하였다. 이 연구결과는 진주종의 새로운 병태생리를 밝혀내는 데 도움이 될 것으로 기대한다.

    Background and Objectives
    Cholesteatoma poses a significant challenge for otolaryngologists, as it has not yet been entirely eradicated through surgical treatments, even with complete surgical excision. There is still no precise understanding of the pathophysiology of cholesteatoma, and research on the associated miRNAs is also deficient. We demonstrated the expression of miRNA in normal skin and middle ear cholesteatoma by next generation sequencing (NGS) technology. The profiles of miRNA and relevant pathway of molecular interactions were investigated.

    Subjects and Methods
    Tissue specimens of middle ear cholesteatoma and post-auricular skin were collected from 13 adult patients during middle ear surgery. Total RNA was extracted, and miRNAs expression profiling was analyzed by NGS technology. Functional gene classification to predict target genes and relevant biological pathways was performed using DIANA-microT-CDS and the Kyoto Encyclopedia Gene and Genome database (KEGG) pathways.

    Results
    Totally the expression of 2,588 miRNA was profiled from middle ear cholesteatoma and skin tissue samples. Expression of seventy-six upregulated and 128 downregulated miRNAs were identified in the cholesteatoma samples compared to normal skin (FC≥2 and p <0.05). Ninety-nine differentially expressed miRNAs (FC≥4 and p <0.05) were used to explore biological pathway which involved in the etiopathogenesis of cholesteatoma. The most predicted pathway in cholesteatoma were the ErbB signaling pathway in the upregulated miRNAs group, and ECM receptor interaction in the downregulated group.

    Conclusions
    This study is the first study investigating small non-coding RNA of human acquired cholesteatoma using NGS technique. Therefore, we were able to identify new miRNAs and pathways related to cholesteatoma. The results of this study are expected to be helpful in revealing new pathophysiology of cholesteatoma.
