Background and Purpose: Sleep disorders have been known to be correlated with stroke. However, sleep disorders are not generally evaluated in acute stroke patients. This study investigated the characteristics of sleep disorders observed in patients with acute cerebral infarction through polysomnography(PSG).
Methods: A prospective study was conducted on 50 patients who underwent PSG within 7 days of onset of cerebral infarction. Demographic factors and risk factors for cerebrovascular disease were collected, and the severity and prognosis of cerebral infarction were evaluated. The types of cerebral infarction were classified using the Trial of Org 10172 in Acute Stroke Treatment(TOAST) classification, and were divided into anterior circulation and posterior circulation according to the location of lesion. Sleep apnea was defined as apnea-hypopnea index(AHI) of 5/h or higher.
Result: Patients diagnosed with obstructive sleep apnea(OSA) were 29(58%) in this study. In the group in which OSA was observed, the prevalence of obesity and the rate of posterior circulation infarction were high. AHI was significantly higher in posterior circulation infarction(mean 18.19) than in anterior circulation infarction(mean 9.70). Smoking and drinking did not correlate with OSA prevalence.
Conclusion: Active PSG for diagnosing sleep disorders including obstructive sleep apnea in patients with acute cerebral infarction may be clinically useful.
