지역사회 중심 가정방문 작업치료사의 직무분석 및 가이드라인 개발 연구 : 보건소 방문재활 작업치료사의 직무를 중심으로 = A Study on Job Analysis and Guidelines Development of Community Based Home Visit Occupational Therapists : Focusing on the Job of Home Visit Occupational Therapists on Public Health Centers

This study aimed to perform job analysis and develop guidelines for community based home visit occupational therapists. The job analysis of the home visit occupational therapist was performed using the DACUM technique. The definition, duties, tasks, and task elements of community based home visit occupational therapists were drawn through a DACUM workshop with 7 home visit occupational therapists on public health centers. The results were validated for 20 home visit occupational therapists on public health centers. Finally, 8 duties, 29 tasks, and 92 task elements were derived based on the content validity ratio. The importance, difficulty, and frequency of each task were investigated in 30 home visit occupational therapists on public health centers, and 22 core tasks were derived using a tree structure method. In order to provide a clinical basis appropriate for the job of a home visit occupational therapist, guidelines were developed based on the guidelines for development and adaptation. 3 guidelines and 17 articles with qualitative evaluation and 49 recommendations were drawn. The core recommendations were a client centered approach and an occupation based approach. The final job analysis and guidelines of community based home visit occupational therapists were developed through expert advice. The guidelines were validated, and most of the recommendations were applicable in clinical practice. The job analysis and guidelines of community based home visit occupational therapists derived from this study provide them to understand the scope of the job easily, and to assist in implementing the evidence based practice in the community. In addition, this study is expected to be used as basic data for additional research on job scope and fees, such as home visit occupational therapy of National Health Insurance and Long-Term Care Insurance, which are expected to be activated in the future.

국문 초록 (Abstract)

본 연구는 지역사회 중심 가정방문 작업치료사의 직무를 분석하고, 근거 기반의 임상 수행을 위해 가이드라인을 개발하고자 하였다. 직무분석은 데이컴 기법으로 수행하였다. 보건소 소속 가정방문 작업치료사 7명을 대상으로 데이컴 워크숍을 통해 지역사회 중심 가정방문 작업치료사의 직무 정의, 책무, 작업, 작업요소를 도출하였다. 도출된 결과에 대해 보건소 소속 가정방문 작업치료사 20명을 대상으로 타당성 검증을 실시하였다. 내용타당도비율에 근거하여 최종적으로 8개의 책무, 29개의 작업, 92개의 작업요소가 도출되었다. 보건소 소속 가정방문 작업치료사 30명을 대상으로 각 작업에 대한 중요도, 난이도, 빈도를 조사하였고, 나무구조도 방법을 이용하여 22개의 핵심 작업을 도출하였다. 지역사회 중심 가정방문 작업치료사의 직무에 적절한 임상적 근거를 제공하고자 수용개작 지침에 근거하여 가이드라인을 개발하였다. 3개의 가이드라인과 17편의 문헌에 대해 질적인 평가를 수행하고, 총 49개의 권고사항을 도출하였다. 핵심적인 권고사항은 클라이언트 중심의 접근과 작업 기반의 접근이었다. 전문가 자문을 통해 최종 지역사회 중심 가정방문 작업치료사의 직무분석 및 가이드라인을 개발하였다. 가이드라인에 대한 타당성 검증 결과, 대부분의 권고사항이 임상에서 적용 가능한 것으로 사료된다. 본 연구에서 도출된 지역사회 중심 가정방문 작업치료사의 직무분석과 가이드라인은 지역사회에서 가정을 방문하는 작업치료사에게 직무의 범위를 쉽게 이해할 수 있고, 근거 기반의 직무를 수행할 수 있도록 도움을 제공한다. 앞으로 활성화가 기대되는 국민건강보험 및 노인장기요양보험 등의 가정방문 작업치료에 관한 직무 범위 및 수가체계에 관한 추가적인 연구에 기초자료로 활용될 수 있을 것으로 기대한다.
