간호사 업무 경험 개선을 위한 여정 분석 연구 : 업무중단과 의사소통을 중심으로

This study analyzed potential problems that occur during the nursing work for the purpose of improving the nurse experience. Because the nurse work is immediately feedback on the patient safety, the working environment of a nurse is very important. However, the nurse experiences workload due to interruption and communication. Therefore, the study should be conducted to improve efficiency and minimize errors in the nursing work associated with patient safety. The observational study was conducted on a total of 9 nurses with the duty (Day, Evening, Night). The nursing work was recorded by the eye-tracker, and the video (56.65 hours) was collected. Based on the observational study data, the framework for analysis of interruption and communication was established to analyze the workload. In addition, based on the nurse experience (task, environment, and organization), the errors and inefficient tasks were derived. Finally, 7 key Pain-points were identified. Nurse journey maps were used to visualize the discovered potential problems of the nursing work, and service blueprints were drawn up to explore opportunities for improvement on processes of complex wards. This study can be used as a basic data for building a system for a nurse centered design for improving patient safety. Besides, based on the results of this study, the medical institutions will be able to provide for the quality of medical services.

국문 초록 (Abstract)

본 연구는 병동 내 환자 안전 증대를 목적으로 의료 서비스의 질을 향상시키기 위해 간호업무 수행 중 발생하는 잠재적인 문제를 분석하고, 재설계하여 간호사 업무 경험에 대한 개선의 기회를 제시하고자 한다. 간호사의 업무 수행은 환자의 안전에 즉각적으로 피드백되기 때문에, 간호사의 근무환경은 매우 중요하다. 그러나 간호사들은 1인당 담당할 환자가 많고, 과도한 업무량, 업무 체증 등으로 인해 각종 의료사고 등 환자 안전에 위협을 미칠 수 있다. 따라서 환자 안전과 관련된 간호사의 업무의 효율성 증진 및 오류 발생 가능성 최소화 연구는 수행되어야 한다. 본 연구는 신촌 세브란스 병동에서 실제 간호업무를 수행하는 간호사를 대상으로 관찰 연구를 수행하였다. 관찰 연구는 간호업무 3교대(Day, Evening, Night) 각 3명씩 총 9명의 간호사를 대상으로 진행하였다. 간호사가 실제 업무를 수행하는 전체 과정을 시선 추적 기기(eye-tracker)를 통해 녹화하였으며, 총 56.65시간의 간호사 업무 수행 영상이 수집되었다. 관찰 연구 데이터를 바탕으로 업무중단과 의사소통의 분석 프레임워크를 구축하여 간호사의 workload를 분석하였으며, 업무, 환경, 조직 3가지 요인의 간호사 경험에 기반하여 간호업무 수행 시 발견되는 오류 비효율적인 업무를 확인하였다. 최종적으로는 이를 종합하여 7가지의 핵심 Pain-point를 도출하였다. 발굴된 간호업무의 잠재적인 문제를 총체적으로 시각화하기 위해 간호사 여정 지도를 활용하였으며, 복잡한 병동 내 프로세스에 대한 개선의 기회를 발굴하기 위해 서비스 청사진을 작성하였다. 본 연구는 간호업무 개선을 위해 간호사의 Workload 차원에서 잠재적인 문제 상황에 대해 분석하였으며, 간호사 경험 관점에서 대안을 구성하였다. 이는 환자 안전 증대를 위한 간호사 중심의 시스템을 구축하는데 기초적 자료로 활용할 수 있을 것이며, 의료 서비스 질 향상에 기여할 수 있을 것으로 사료된다.
