Societies change as population structure is continually altered, and birth cohort is one of the concepts that explain such social change. A birth cohort is a group of people who were born on the same year or period and thus age together. People who were born in different years, that is, different birth cohorts, grow in varying socio-historical and dynamic contexts, which result in differences of social dispositions and physical abilities.
This study used APC analysis method to establish explanatory models on healthcare expenditure in Korea reflecting birth cohort factor. Based on these models, we tried to investigate the effects of ageing population on future healthcare expenditure by identifying changes in future healthcare expenditure through simulation by scenarios.
APC IE analysis results show that age, period, and cohort effects significantly affect healthcare expenditure per capita with National Health Insurance. Age showed the greatest effect. The coefficient of cohort effect was not as high as age effect, but showed greater influence than period effect. Cohort effect, excluding some cohort groups, healthcare expenditure decreases in those born in more recent years. The cohort effect observed in these study findings can be interpreted to show ‘healthy ageing’ phenomenon.
Forecasting results based on age, period and birth cohort effects on healthcare expenditure, pure demographic effect shows annual average increase of 1.6~1.8% in healthcare expenditure due to ageing population from increase in life expectancy. Healthy ageing effect shows annual decrease effect of -1.74~-1.57% in healthcare expenditure.
Forecasting results from the model using cohort(APC) analysis method estimated lower healthcare expenditure than the model that did not consider cohort effect. This suggests age and period effects can be overestimated if ‘cohort effect’ is not controlled. Estimated future population by National Statistical Office are calculated for population changes due to birth and death, reflecting ageing population consideration. If future population estimations are used in analysis model that estimates healthcare expenditure per capita to apply population by groups, ageing population factor can be duplicative. Therefore, in analysis model that applies future population, it needs to examine whether ageing population is duplicate.
Conclusions obtained from this study are as below. First, since the influence of population factor itself on healthcare expenditure increase is not absolute. the increasing trend of healthcare expenditure should be acknowledged and alternatives should be developed to increase the value of healthcare expenditure. Second, there is a need for active policy developments to improve health of the people to allow stronger intensity of healthy ageing effect. There is urgent need of paradigm shift from curative care centered to prevention centered healthcare service. Third, there is a need to use a factor that reflects social change in healthcare expenditure analysis such as birth cohort.
Ageing population is acknowledged as significant risk factor in future healthcare system. However, the current healthy ageing effect can be seen as a hopeful signal. Using the fear of ageing population as an opportunity to improve our current system can further facilitate sustainability of healthcare system.
