연구의 배경: 계절과 월경주기에 따른 기분과 행동의 변화는 다양한 기분장애들에서 보고되어 왔다. 그러나, 이러한 두 가지 주기성 변화가 양극성 장애에서 임상 삽화와 무관하게 생애전반의 특질로서 높게 나타나는지 여부와 이들이 서로 연관되어 있는지에 대해서는 잘 알려져 있지 않다. 본 연구에서는 제 1형과 제 2형 양극성 장애 여자 환자에서 계절성과 월경전 증상을 정상군과 비교하고 두 특질 간의 연관성을 조사하였다.
연구 방법: 임상적으로 안정된 상태에 있는 양극성 장애 여자 환자들 61명(1형 30명, 2형 31명)과 나이로 짝지은 건강한 여자 대조군 122명을 대상으로 계절성 변화 평가 설문(Seasonal Pattern Assessment Questionnaire)과 월경전 증후군 평가 도구(Premenstrual symptoms screening tool)를 작성하도록 하였다. 생애 전반에 기초하여 후향적으로 측정된 전반적 계절성 및 계절성 정동 장애(seasonal affective disorder)와 월경전 불쾌장애(premenstrual dysphoric disorder)의 빈도를 구하여 세 군에서 비교하였다.
연구 결과: 1형 환자군에서 PMDD와 SAD에 해당하는 비율은 6.7%와 20.0% 였으며, 2형 환자군에서는 22.6%와 29.0%였으며, 정상군에서는 1.6%와 3.3%였다. 1형과 2형 양극성 장애 환자군에서 정상군보다 평균 계절성 점수가 높게 나타났다(p<0.0001). 2형 양극성 장애 환자군에서 1형 환자군보다 계절성과 월경전 증상이 모두 높았다. 중등도 이상의 PMS가 있는 군이 PMS가 없거나 약하게 경험하는 군에 비해 전반적 계절성 점수에서 유의하게 높은 값을 보였다(p<0.0001). 양극성 장애 환자군에서 계절성과 월경전 증상간에 유의한 상호연관성이 있었으며, 정상군에서도 이러한 상호연관성이 나타났다. 계절성과 월경전 증상간의 연관성 정도는 환자군에서 정상군에 비해 유의하게 높았다(p=0.029).
결론: 본 연구의 결과는 양극성 장애 여자 환자들이 기분 삽화와 관계없이 월경주기와 계절에 따른 기분과 행동의 변화를 더 자주 경험한다는 것과, 계절성과 월경전 증상이 서로 연관되어 있음을 보여주고 있다. 따라서 이 두 주기성 상태가 공유하는 생물학적 기전과 이에 대한 취약성이 양극성 장애의 주기성 상태의 발현과 관련이 있다고 볼 수 있겠다.

Background: Although seasonal affective disorder and premenstrual syndrome (PMS) are frequently comorbid with mood disorders, It is unknown whether lifetime traits of seasonality and premenstrual distress are related to bipolar disorder independent of mood episodes. This study investigated these two cyclic conditions in patients with bipolar I and bipolar II disorders, evaluating the association between them.
Methods: Subjects included 61 female patients with either bipolar I or bipolar II disorder, and 122 healthy women. Seasonality and premenstrual symptoms were measured retrospectively on a lifetime basis using the Seasonal Pattern Assessment Questionnaire (SPAQ) and the Premenstrual Symptoms Screening Tool (PSST)
Results: Patients showed higher global seasonality scores on the SPAQ compared to normal controls. Further to this, the patient-control difference was more prominent in cases of bipolar II disorder (p<0.0001) than it was in bipolar I disorder (p=0.001). The prevalence of moderate to severe PMS as indicated on the PSST was also signifcantly higher in bipolar II disorder patients(51.6%) as compared to controls(19.7%). A significant association between seasonality and PMS was observed in both patient and control groups, although it was more prominent in the patient group.
Conclusions: The results suggest that female patients with bipolar disorder experience seasonal and premenstrual changes in mood and behavior regardless of their mood episodes, and those traits of seasonality and PMS are associated with each other. A common biological mechanism and the vulnerability to these two cyclic conditions may be involved in the development of the cyclicity of bipolar disorder.
