2006년 11월부터 2007년 5월까지 중앙대학교 용산병원에 급성 위장관염으로 입원한 환아들의 분변으로부터 일련의 RT-PCR과 multiplex PCR에 의하여 로타바이러스의 G 혈청형과 P 유전자형을 조사하였다. 로타바이러스 양성으로 판정된 분변 시료 79례에서 나타난 G 혈청형은 G1이 38례 (48.0%)로 가장 많았으며, G2가 17례 (21.5%), G3이 12례 (15.1%), G4가 4례 (5.1%), G8과 G9가 각 1례 (1.3%) 검출되었다. 중복 감염은 5례 (6.4%) 검출되었으며, 1례의 시료 (1.3%)는 혈청형을 결정할 수 없었다. P 유전자형은 P[8]이 25례 (31.6%)로 가장 많았으며, P[4]가 22례 (27.8%), P[6]이 16례 (20.3%), P[9]가 2례 (2.5%) 검출되었다. 중복 감염은 5례 (6.4%) 검출되었으며, 9례의 시료 (11.4%)는 유전자형을 규명할 수 없었다. G 혈청형과 P 유전자형의 조합형은 G1P[4]가 14례 (17.8%)로 가장 많았으며, G1P[8]이 12례 (15.1%), G2P[6]이 8례 (10.1%), G2P[8]이 6례 (7.6%), G1P[6], G2P[4], G3P[4] 및 G3P[6]이 각 3례 (3.7%), G3P[8]과 G4P[4]가 각 2례 (2.5%), G1P[9], G3P[9], G4P[6], G4P[8] 및 G9P[8]이 각 1례 (1.3%) 검출되었다. 중복 감염은 8례 (10.1%) 검출되었으며, 어느 한 쪽이 결정되지 않았던 경우는 10례 (12.7%)로 나타났다. 간접형광항체법을 시행하여 MA104 세포에서 로타바이러스의 감염 여부를 확인하였으며, 국내 로타바이러스 분리주 CAU 016-07과 CAU 037-07의 RNA 전기영동상은 "long" RNA 패턴을 보여 아군 Ⅱ로 판명되었다. 외국 유래주들과의 유전적 상관성은 VP7의 경우 2006년 스페인에서 보고된 5.1주와 유전적으로 가장 밀접하였고, NSP4의 경우 유전자형 B에 속하였다. 이상의 결과를 종합해 볼 때, 본 연구 결과는 국내에 유행하는 로타바이러스에 대한 분자역학적 기초 자료를 제공함과 동시에 병원성 관련 연구와 백신 개발 전략 수립에 유용하게 사용될 것으로 사료된다.

Seventy nine group A rotavirus antigen-positive stool specimens were collected from young children admitted to Chung-Ang University Yongsan Hospital with a diagnosis of acute gastroenteritis between November 2006 and May 2007. G and P typing was carried out by RT-PCR and multiplex PCR. The most prevalent G serotype was G1 (48.0%), followed by G2 (21.5%), G3 (15.1%), G4 (5.1%), G8 (1.3%) and G9 (1.3%). Mixed G types were indentified in five specimens (6.4%) and one specimen (1.3%) remained untypeable. Among P genotypes, the most common strain was P[8] (31.6%), followed by P[4] (27.8%), P[6] (20.3%) and P[9] (2.5%). Mixed P types occurred in five specimens (6.4%) and nine specimens (11.4%) remained untypeable. The predominant G and P type combination was G1P[4] (17.8%), followed by G1P[8] (15.1%), G2P[6] (10.1%), G2P[8] (7.6%), G1P[6] (3.7%), G2P[4] (3.7%), G3P[4] (3.7%), G3P[6] (3.7%), G3P[8] (2.5%), G4P[4] (2.5%), G1P[9] (1.3%), G3P[9] (1.3%), G4P[6] (1.3%), G4P[8] (1.3%) and G9P[8] (1.3%). Mixed infections were detected in eight specimens (10.1%) and either G or P type could not be determined in ten specimens (12.7%). Two Korean rotavirus isolates, CAU 016-07 and CAU 037-07 were confirmed by indirect immunofluorescence assay. RNA electropherotypes of these two strains showed "long" RNA pattern which is a specific character of group A rotavirus subgroup Ⅱ. Phylogenetic analysis revealed that the VP7 genes of two Korean isolates were most closely related to that of a Spainish strain 5.1 reported in 2006 and the NSP4 genes of two Korean isolates belonged to NSP4 genotype B. The findings of this study will be useful for molecular epidemiological studies on Korean rotavirus strains and for future rotavirus vaccine development.
