    의약품명 유사로 인한 LASA(Look-Alike, Sound-Alike) 오류는 처방·조제·투약 전 과정에서 발생할 수있으며, 발음·철자 유사뿐 아니라 포장·라벨·전산목록(UI) 등 사용 맥락 요인이 결합할 때 위험이 증폭된다. 본 연구는 LASA 오류의 위험요인을 체계적으로 분해하고, 의약품 제품명 심사와 상표심사의 제도적 연계를 통해 예방 가능성을 높이기 위한 개선 방향을 제시한다. 이를 위해 ISMP 혼동약품명 목록(2024), ISMP Canada, FDA MedWatch, 국내 환자안전보고(KOPS) 및 국내 보고자료를 기반으로 오류사례 1,425쌍(1차 DB)을 구축하고, (1) 명칭쌍 중복 제거(A-B=B-A), (2) 공백·하이픈·대소문자 표기 통일, (3) 제형·함량 표기 차이가 혼동의 핵심인 경우 맥락요인으로 별도 표지하는 정규화 규칙을 적용하였다. 오류 발생 단계는 처방·조제·투약으로 분류하고, 오류유형은 철자·발음·맥락의 3축으로 다중코딩한 뒤, 단계별 조건부 분포(P(유형|단계))를 산출하였다. 분석 결과, 처방 단계에서는 발음·철자 유형의비중이 상대적으로 높고, 조제 단계에서는 철자 유형이 두드러지며, 투약 단계에서는 맥락 요인의 비중이 확대되는 경향이 확인되었다. 또한 해외 주요국은 POCA 등 정량 도구를 활용한 사전 명칭심사와시판 후 오류 보고·완화조치의 환류 체계를 결합하여 LASA 위험을 관리하는 반면, 한국은 허가 단계 제품명 심사와 상표심사가 분절되어 LASA 위험을 포섭하는 데 제도적 공백이 존재한다. 이를 바탕으로한글 음운·자모 체계를 반영한 한국형 POCA(K-POCA) 도입과 고위험 후보군의 맥락평가 절차 표준화, 상표심사에 환자안전 관점(성분 오인혼동)을 반영하는 기준 정합화, 그리고 시판 후 오류 보고가 심사및 사후관리로 반영되는 연계 설계를 제안한다. 본 연구는 LASA 오류를 환자안전 문제이자 지식재산· 규제과학의 교차 관점에서 분석하고, 사례 DB, 유형·단계 분포, 제도 비교, 정책 제언을 연결하는 통합적 분석틀을 제시하였다는 점에서 학문적·실무적 의의를 지닌다.

    Medication name similarity–related look-alike, sound-alike (LASA) errors occur across prescribing, dispensing, and administration. These risks particularly increase when phonetic/orthographic similarity interacts with contextual factors, such as packaging, labeling, and computerized medication lists. This study decomposed the LASA risk factors, and proposed prevention policy measures through linkages between regulatory drug name reviews and trademark examination. We constructed a database of 1,425 error pairs from ISMP sources (2024) and other reports (e.g., FDA MedWatchand KOPS). The data were normalized by removing order-invariant duplicates (A–B = B– A), standardizing basic name formatting, and tagging formulation/strength-driven confusions as contextual. Stages were classified as prescribing, dispensing, and administration; types were multi-coded as orthographic, phonetic, or contextual; and stage-conditional distributions (P(type|stage)) were estimated. The results showed higher phonetic/orthographic shares in prescribing, a dominant orthographic share in dispensing, and an increased contextual share in administration. Comparative analysis suggested that major jurisdictions combine quantitative pre-market name review(e.g., POCA) with post-market reporting and feedback. Meanwhile, Korea faces gaps due to the separation of approval-stage name reviews and trademark examinations. We proposed a Korean-adapted POCA (K-POCA) reflecting Hangul phonology and grapheme structure; standardized contextual review for high-risk candidates; trademark criteria aligned with patient safety (ingredient-confusion risk); and a linkage mechanism that feeds post-market error reports into review and post-market management. Overall, this study offers an integrated framework that links a case database, type–stage distributions, institutional comparisons, and policy recommendations at the intersection of patient safety, intellectual property, and regulatory science.
