한국 치과위생사 제도 관련 연구 동향(2015-2024): 주제범위 문헌고찰 = Research Trends of the Dental Hygienist System in Korea (2015-2024): A Scoping Review

국문 초록 (Abstract)

연구배경 및 목적: 국내 치과위생사 제도는 의료기사법을 근거로 업무 범위가 규정되어 왔으나, 이는 임상에서 실제로 수행되는 다양한 업무를 보장하지 못하고 있다. 이에 의료법 개정, 전문치과위생사 제도 등 다양한 제도 적 논의가 이루어지고 있으나, 제도의 전반적인 변화와 한계를 종합적으로 분석한 연구는 미비하다. 본 연구는 최근 10년간 국내 치과위생사 제도 관련 문헌을 주제범위 문헌고찰로 검토하여, 주요 쟁점과 개선 방향을 도출하고자 한다.
방법: Arksey와 O’Malley가 제안한 절차에 따라 문헌을 선정하여, 총 32편을 분석하였다. 문헌 내용 중 핵심 진술을 기반으로 코딩하여 하위범주를 도출하고 이를 핵심 연구주제로 재구조화하였다.
결과: 총 25개의 하위범주가 도출되었으며, ‘법·제도 기반의 업무범위·자율성 재정립’, ‘전문 치과위생사 제도 도입·운영의 필요성’, ‘인력정책·고용환경 정비’, ‘제도에 대한 인식·사회적 수용성’, ‘서비스 제공영역 확장과 돌봄·지역사회 기반 실천’의 핵심 연구주제로 구조화되었다.
결론: 치과위생사 제도는 법·제도 정비, 전문성 보장, 인력정책 개선, 제도 인식 제고, 서비스 영역 확장이라는 다층적 과제에 직면해 있으며, 실제 수행 직무와 전문성을 반영한 업무범위 재정립, 근로환경 개선, 사회적 수용성 확보, 지역사회 구강보건서비스 핵심인력으로서의 역할 확장이 요구된다.

다국어 초록 (Multilingual Abstract)

Background: In Korea, the scope of practice for dental hygienists has been defined under the Medical Technicians Act, yet these regulations do not fully reflect the diverse tasks performed in clinical settings. Although various institutional discussions—such as amendments to the Medical Service Act and the introduction of specialist dental hygienist systems—have emerged, comprehensive analyses addressing the broader structural changes and limitations of the system remain limited. This study aims to review literature from the past decade to identify key issues and directions for improving the dental hygienist system.
Methods: Using the framework proposed by Arksey and O’Malley, 32 documents related to the dental hygienist system were selected. Key statements were coded to derive subcategories, which were subsequently reorganized into major thematic areas.
Results: Twenty-five subcategories were extracted and consolidated into five core themes: redefining the legal and institutional basis for scope of practice and autonomy; the need for implementing and managing specialist dental hygienist systems; improving workforce policies and employment conditions; enhancing awareness and social acceptance of the system; and expanding service domains toward community-based and care-oriented practices.
Conclusions: The Korean dental hygienist system faces multidimensional challenges, including legal refinement, strengthened professional recognition, improved workforce policies, greater institutional and public awareness, and broader service domains. To address these issues, it is essential to realign the scope of practice with actual clinical responsibilities and professional competencies, enhance working conditions, secure social acceptance, and expand the role of dental hygienists as key personnel in community oral health services.
